will be responsible for choosing their own

| 3.

If
a team member experiences an OSHA recordable
accident, that team restarts its' consecutive
streak at one (1) day. |
 |
|
| 4.
|
Part
time and temporary employees will be included in
this program but awards will be paid
proportionally according to their hours worked
divided into a standard work period. |
 |
|
| 5.
|
New
employees are eligible for quarterly awards on a
"proportional" basis. New employees
must be on the payroll for six (6) months to
become eligible for any annual bonus drawings. |
 |
|
| 6.
|
Employees
off on a job-related injury are ineligible for
the quarterly awards; when these employees return,
they are eligible for the quarterly awards on a
"proportional" basis. After returning,
these employees will need to complete six (6)
months on the job to become eligible for any
annual bonus drawings. |

| 7.
|
Employees
off on an approved leave of absence are eligible
for quarterly awards on a
"proportional" basis. They are
eligible for any bonus drawings if they've been
on active service for six (6) months or more for
the year's drawing. |
 |
|
| 8.
|
Management
reserves the right to investigate all accidents,
modify or add rules as necessary and to
terminate or extend this program at will. The
operations director and human resources manager
will be the administrators of this program. |
